Breast implants (silicone gel or saline solution)
Silicone implants with cohesive gel are the most widely used because they feel very natural and are dimensionally stable.
Saline implants are used less frequently.
The implants can be placed either under the pectoral muscle or over the pectoral muscle (submuscular vs. subglandular).
There are round and anatomical (teardrop-shaped) implants.
Autologous fat transplantation
- The body's own fat is extracted by liposuction from another part of the body, processed and injected into the breast.
- Suitable for moderate magnification and a very natural shape.
Durability of transplants
Modern breast implants are very durable. Whereas in the past, replacement was often recommended after around 10 years, experts now believe that high-quality implants can last 15 to 20 years or even longer, provided no complications occur.
Replacement is only necessary if:
complications may occur (e.g. capsular fibrosis, rupture/tear of the implant).
The patient wants a change for aesthetic reasons (e.g. different size or shape).
Regular medical check-ups (e.g. ultrasound) are important to monitor the condition of the implants.
Breast reduction methods
I-cut method (vertical method)
Incision around the areola and a vertical incision from there to the underbust crease.
Leaves fewer scars than the T-cut method.
T-cut method (anchor cut)
Incision around the areola, vertically downwards and an additional horizontal incision along the underbust crease.
Enables the greatest reduction, but leaves the most visible scarring.
Benelli method (periareolar)
Cut only around the areola.
Only suitable for minor reductions and tightening.

Pseudogynecomastia (fat breast, lipomastia)
Pseudogynecomastia is an enlargement of the male breast due to pure fat storage and without an increase in mammary gland tissue.
Main cause: Severe overweight or obesity. The excess fat is stored throughout the body, including in the breast area.
